Identification Recommendation

To parents who wish to nominate their child for Gifted/Talented identification

Thank you for your interest in Gifted/Talented (G/T) services for your student.  We make determinations for Adams 12 Gifted/Talented services based on an entire body of evidence so that we may best serve the students in our district.  The information you provide to us is a critical part of the body of evidence we will consider in evaluating your student’s nomination for gifted and talented services.  We appreciate you taking the time to provide us with the needed information as thoughtfully and thoroughly as possible.

GT Areas of Strength

When completing the Identification Recommendation form, please see the following areas of giftedness, click on the link  of Strength, and choose the area that best fits the student you are recommending for GT identification.  For more detailed information on these areas, click here.

Please complete the Parent Identification Recommendation form  and the Parent Rating Scale below and return it to either: your childs teacher, building GT Coordinator, email to Cathy.Jordan@adams12.org, or fax to 720-972-3880.

Complete the electronic Identification Recommendation form.  Save the form under a new name. 

Directions on saving the PDF Form.  Click - File in the tool bar, Select - Save As, Change the file name, if you don’t the pdf will not save.  Hit save.

Parents should email Cathy.Jordan@adams12.org or fax: 720-972-3880 the completed form at the district GT office.

All Parent Identification Recommendation forms must be submitted to the District GT office at least 2 weeks prior to the scheduled building testing date. Please check with your school's GT coordinator for the scheduled testing date.

Identification decisions will be made within 6 weeks from the testing date.
